Subject: Many Nebraksa Schools and Libraries must file for reimbursment
Nebraska schools and libraries stand to lose up to $2,257,287.38 in
already approved E-rate funding if Billed Entity Applicant Reimbursement
(BEAR) forms are not filed in a timely manner.
The 2005-2006 Erate funding year ended July 1st, and many Nebraska
schools and libraries are listed as needing to file Form 472 BEARs in
order to be reimbursed for funds they have already paid to service
providers from July 2005 through June 2006.
If your school or library is on the list below, you were approved for
funding and filed Form 486 Receipt of Service Confirmation, but the
Invoicing Mode (NOT SET) indicates you did not receive credits on your
bills and have not yet filed a reimbursement request.
It's possible that some of the services listed were never used or paid
for. If so, no reimbursement is allowed. But, if your school or library
used and paid for the services listed you will need to file Form 472,
Billed Entity Applicant Reimbursement, to receive your approved funding.
Form 472 must be postmarked or received no later than 120 days after the
last date to receive service or no later than 120 days after the date of
the Form 486 Notification Letter, whichever is later. For most
applicants, the deadline to file a BEAR for the 2005-2006 funding year
is October 28, 2006 but the sooner you file, the sooner you will
receive your reimbursement check. When you receive your June service
provider invoice, you may file for reimbursement for funds you have
already paid for any approved funding requests since July 2005, but you
may not request reimbursement for more than your approved committed
amount for any given service.
The Form 472 BEAR is a paper form that must be mailed to the SLD but you
may complete it online, and print it using Adobe Reader at:

The NE-ERATE listserv does not allow attachments like spreadsheets but I
hope the table below will be formatted correctly for easy reading. If
not, do a data retrieval for your own school or library at:
http://www.sl.universalservice.org/funding/opendatasearch/Search1.asp
to get the same information.
Good luck with your reimbursement requests and please do not lose E-rate
funding by failing to file one last required form for the Erate funding
