Please check the list below. If you are on it and need to file any Form
486's you need to do so as soon as possible so as not to risk losing
partial funding. If you have already filed the 486 on these
applications then please disregard this notice. I will be checking on
those that are in the consortium.

Subject: Form 486 due dates
Included below is a table of Nebraska schools and libraries that have
had services funded for the current E-rate funding year 2006-2007, (as
shown by a Funding Committment Decision Letter date) but are not yet in
the SLD database as having filed Form 486, "Receipt of Service
Confirmation".
The Form 486 is due 120 days after receipt of the FCDL or 120 days after
the service start date, whichever is later and must be filed before you
may receive discounts or reimbursements. Applicants do not lose their
funding entirely but are penalized proportionally for each day past the
486 due date that the Form 486 is late.
If your Form 486 is shown as being past due, it's possible you actually
did file on time but your information has not yet been entered into the
SLD database. The data shown is as of today November 27, 2006.
You will notice that applicants who were funded this month have until
March to file form 486 but no credits or refunds can be authorized until
the 486 is received so you should file it as soon as you are able to
make the certifications required on that form after receipt of your
