[E10-erate] RE: Qwest Credits for Erate - SAVE THIS INFORMATION!

I have had several of you ask about the procedure for getting credits
from Qwest on your erate applications.  
 
You must e-mail or Fax them EVERY YEAR after you have filed your Form
486 and request that the funds be credited to your accounts.
You need to send them the following:
    Applicant Name
    Application Number
    Funding Request Number 
    Account number(s) you want credited.
 
FAX that information to: 
E-Rate Dept.   1-888-393-9997
Or
E-mail it to: erate at qwest.com
