[E10-erate] Erate documentation still needed!

REMINDER!

We still have not received the Form 479 and Letter of Agency from
several of our schools.  We MUST have these forms for ESU 10's
documentation purposes from each school district, regardless of  whether
you file for Erate discounts, and we must have them on file each year.  

Below is a link to each of these forms.  You can click on the link, fill
the form out on your computer, print it off, sign it and return it to
either Sue or Stacy at ESU 10.  If you have questions you can contact
either of us at erate at esu10.org or call us at 308-865-5664.
