If you have not yet filed your FY2008 Form 470, don't send it in on
paper - file it online. Thursday, January 10, 2008, is the LAST DAY TO
POST a Form 470 to the USAC website and still be able to complete all
competitive bidding requirements and file a Form 471 before the close of
the filing window.

If you have completed the 28-day requirement on your Form 470 and still
need to file your Form 471 ESU 10 will have our lab open all day on
January 23rd. Sue Rains and Stacy Peters will be available to assist you
with filing your E-rate forms.  Please register for the workday so we
have an idea of the number of participants.  Feel free to come and go as
you need.  Remember the Deadline for filing your Form 471 is 10:59 p.m.
CST February 7, 2008.

 

*        PIN Number for online Erate filing (we will not be able to
submit and certify your forms if you do not have this with you)

*        Copy(ies) of last year's Form 471 - With a few exceptions most
of the information you need can be found on your previous years form. 

*        Service Provider Invoice Amounts

*        Service Provider Identification Number (SPIN)

*        Expiration Dates for any contracts that have been signed.

*        Student Count & Free & Reduced Lunch Information (If you have
ANY Pre-K students listed on your membership report please contact us
ahead of your scheduled appointment)

*        Internet Access Service Speed

*        Number of Buildings served with Internet Access

*        Number of Classrooms with Internet Access

*        Direct Connections to the Internet: Number of Drops

*        Number of Computers with Internet Access

*        Description of Services from the Service Provider.
