[E10-erate] E-rate Starts Funding for FY2008 - What's the NEXT

Good Morning Everyone,
In case you aren't aware the SLD has started sending out Funding
Commitment Decision Letters to schools.
When you receive your Funding Commitment Decision Letter (FCDL), JUST
DON'T FILE IT!!! Receipt of your FCDL triggers the need for the next
required form, Form 486 "Receipt of Service Confirmation". Applicants
cannot receive discounts or reimbursements on funded services until the
SLD receives and approves a completed Form 486.
Form 486 is now available for online filing for the 2008-2009 funding
year on the SLD website at http://www.sl.universalservice.org/menu.asp
Applicants may file Form 486 "early" (after receipt of the FCDL but in
advance of services starting) if certain conditions are met. If the Form
486 is filed in this manner, it must be received by USAC or postmarked
on or before July 31 of the Funding Year.
You may be eligible to file the Form 486 on or before July 31 of the
Funding Year if:
* you have received your FCDL and
* you have confirmed with the named service provider that the services
you ordered will start on the Service Start
Date, which must be in July of the Funding Year and
* you are able to accurately make all of the relevant certifications in
Block 4 in advance of the date of the start of
discounted services. (i.e. CIPA certification, Tech Plans and etc.)
IF you signed an Erate Support Services Agreement with ESU 10 you MUST
make sure that Stacy or I receive any and ALL copies of correspondence
that you receive from the SLD. We CANNOT complete the next step for you
if we do not have copies of this information.
