[E10-erate] Erate Forms for FY2009 (YES...ALREADY!)

Hello All,

 

Stacy and I are going to try very hard to get a 'jump' on the MOUNDS of
paperwork for Erate for Funding Year 2009.   You can now find the
following forms for Funding Year 2009 on our Erate website at:

FILES FOR ERATE!

 

We would greatly appreciate it if you could fill out ALL of these forms
for FUNDING YEAR 2009 and return them to us as soon as you can so we can
get started with the documentation process for FY2009.   We will be
sending update Form 479's and requesting School Membership Reports and
Free and Reduced Lunch information in the fall!  So be looking for those
forms sometime in late July or early August!

 

If you have an E-rate Support Services Agreement this year and would
like to renew for next year please fill out the form and return it to
Stacy or I.  Please be sure to select the option you would like for the
coming year.  If nothing has changed on the contact information and
Billed Entity and NCES numbers you can leave those blank.  However, if
there are changes please fill out the correct information and send them
to us.

Please let us know if you wish to be billed in this fiscal year or next
and we will do our best to accommodate you.

We would encourage you to please read over the items in the contract
that your school district is responsible for. Again we would also like
to stress that you must make sure that Stacy or I receive any and all
copies of correspondence that you receive from the SLD.  We are unable
to complete the next step in the application process if we do not have
copies of this information.  We sincerely appreciate you assistance with
this.
